Dr. Trivellini explains, in an interview with Dr. Sara Lea Salas, why the no-shave transplant is a precision tool and not a convenience.

Why operate with long hair
In this interview (Dr. Sara Lea Salas, 2025) Dr. Roberto Trivellini and Dr. Marcelo Pitchon, the two pioneers of the long-hair technique, explain why the no-shave transplant is not a convenience for the patient but a precision tool for the surgeon.
The idea of transplanting while keeping the hair long was born in 2004 with Dr. Pitchon (Belo Horizonte, Brazil). Dr. Trivellini refined the FUE technique, until then performed only with short hair, so that extraction and implantation became possible with long hair, without the patient having to shave.
In Dr. Trivellini's words
It is another universe of hair transplantation. The transplant with shaved hair is very good, that is the first thing to say. But when you do it with long hair, the surgeon sees one hundred percent of what he does.
A transplant is not just placing hair in the skin. The hair has to sit in full harmony with the rest of the existing hair. If you only place it in the skin without seeing it, because everything is shaved, you can end up with areas like a valley, with no hair, and you lose the direction and the angle of the hairs.
To achieve a transplant with the maximum artistic precision, it can only be done with long hair.
Key points
- See the result while you do it. With long hair the surgeon checks in real time the direction, angle and density of each graft relative to the existing hair.
- Harmony is the clinical goal. A technically correct graft placed without seeing the neighbouring hair can leave density "valleys" or angles that do not blend in.
- Long Hair FUE is a technique, not a marketing trick. It requires specific instruments (the long-hair punch of the Trivellini system) and a different workflow from shaved FUE.
- For the patient, the benefit is immediate discretion: they leave the operating room with minimal signs of having been operated on.
