Surgeon pain points
Patients who will not shave the donor area: answering from technique
Many patients rule out FUE because they will not shave. Which technical options exist for the surgeon and what to learn in order to offer them with judgment.

It is one of the most repeated objections in the clinic: "I am interested, but I do not want people to notice I had surgery." For some patients, shaving the donor area is a deal-breaker. For the surgeon, the question is what can be offered without forcing a technique one has not mastered.
What happens when you have no alternative
- The patient postpones or gives up.
- A technique with a linear scar is proposed when they preferred to avoid it.
- Partial FUE or limited shaving is offered, which does not always fit what is needed.
Each of these is a clinical and patient decision, not something a blog should settle. What we can say is that having the technical competence widens what you can propose.
The technical option: Long Hair FUE
Long Hair FUE extracts and implants while keeping hair length, without shaving the donor area. According to the academy's documentation, the cosmetic result can be visible at 24 or 48 hours. It is a more demanding technique than shaved FUE and the literature presents it as emerging, so it is worth learning with method before offering it.
What to learn before offering it
- How to locate the follicle and its angle with long hair.
- How to adapt extraction parameters by layer.
- How to implant long hair without damaging the graft.
- How to inform the patient honestly about limits and timing.
How the academy helps
The online course covers Long Hair FUE and the multiphase system in a module with a quiz. If you already operate, the 1:1 training session lets you review your approach with Dr. Trivellini. It is training: it promises no clinical results and does not license you by itself.
How each market option solves it
- ISHRS webinars: the July 2025 Spanish webinar included talks on long hair FUE [1], as an overview.
- Live surgery and hands-on courses: seeing or practicing the technique requires attending a specific event.
- Manufacturers: training oriented to their device.

Frequently asked questions
- Can unshaven FUE be done on any patient?
- It is a clinical decision for the surgeon with each patient. The academy explains the technique; it does not determine who is a candidate.
